Scholarship Information
The Bret H. Chernik Amputee Foundation was formed in 1998. Bret was an aspiring hockey official who stopped to help a stranded motorist and was hit by a car, which resulted in the amputation of his left leg. In addition to helping Bret with the staggering medical expenses he incurs from the loss of a limb, the Foundation also attempts to assist other amputees improve the quality of their lives.
Thomas M. Demartino, Jr. was also an ice hockey official, active in USA Hockey and the Connecticut Chapter of the National Ice Hockey Officials Association. In May, 2003, Tommy was tragically killed in an automobile accident. It was the desire of his family that all donations in Tommy's memory go the Chernik Amputee Foundation.
To assist aspiring amputee hockey players, the Foundation now offers the Thomas M. Demartino, Jr. Scholarship for hockey training camps or related programs. The scholarship serves to perpetuate the memory of Tommy Demartino by making scholarship awards available to aspiring players and officials. Scholarships may be awarded up to $500, depending on the number of recipients.
Selection for the awards is based on financial need and other considerations. Interested applicants should complete the application available on-line at www.chernikfoundation.org and mail the hard copy with signature no later than July 07, 2008 to the Bret H. Chernik Amputee Foundation, P.O. Box 905, Suffield, CT 06078. No application will have a final review without a signature page.
